Seems like it was probably drawn from scratch with a heavy toke. (Incidentally, Switched on Bach was performed, not programmed. No computers or sequencers involved.)3 -

I can't say how well it works for native readers, but the simple design is probably because it was based on a fairly low PPM bitmap font.6 -
